Reason for filing

The reason for the presentation must be stated, that is:

• Notify the appointment or registration of a representative (box 300 or 350)
• communicate the withdrawal of representative due to revocation or resignation of representation (withdrawal, box 301 or 351) or, otherwise,
• communicate a modification of the representation (modification, box 302 or 352)

The effective date of the cause that motivates the communication must also be indicated (box 303 or 353).

Modifications may only refer to the cause of representation, the type or the title thereof. Any other modification relating to the identifying data or the tax address of the representative must be communicated by the representative himself.
