Form 128

Form 128 must be submitted (box 706 “high” in form 036 and box 726 “Date”) , with the payment of the corresponding amount by the withholding agents and those required to pay into account that satisfy the income or returns on movable capital from capitalization operations and life or disability insurance contracts, obtained by taxpayers for the Personal Income Tax and the income from the same source obtained by taxpayers for the Corporate Tax and by taxpayers for the Non-Resident Income Tax (permanent establishments)
