Form 036: Box 122. Change of tax address (pages 2A, 2B and 2C)

This box and model will be used, depending on the case in question, by:

• Natural persons (form 036) registered in the census of professional entrepreneurs and withholding agents, who change their habitual residence.
• Entities (only model 036) that change the address where their administrative management and the address of their businesses are effectively centralized.
• Non-resident entities (only form 036) that change the address where their administrative management and business address are actually centralized or the registered office abroad that does not coincide with the tax address.
